Error while send

Been 2-3 weeks having this error. But sometimes it is okay, sometimes return this error. I already create new wallet using the same seed, but still cannot send Dash to another address. Please refer the image below for the error details.

error: the transaction was rejected by network rules.

16: mandatory-script-verify-flag-failed (Script failed an OP_EQUALVERIFY operation)

Any fix for this?

- I've tried to change all 4 server. But still the same error exists.

Usually that means that your transaction is malformed.

I quite did not understand. What do you mean by malformed actually?

Are you creating and sending the transaction online, or you have an offline setup?

What outputs are you trying to spend?

Can you PM me the transaction, so I would inspect what's going wrong with it?
