Does masternode location matter? If the server is in the USA or Africa?

Server A in the USA where interest in DASH is greater but there are thousands of masternodes.
Server B is the first masternode in an African country X. Very little interest but you're the only masternode in X

Is the payout the same for both servers?
If there are more masternode services being done by server A, would A receive more DASH in the long run?
Assuming running costs are the same, why would anyone chose to setup in one location over another?

Yes, location doesn't matter and payouts are roughly the same for all masternodes (depends on fees collected in blocks).
We are aiming to use number of served requests in masternode reward calculation but that's something to be implemented in the future.
One would chose one location over another due to his own preferences and maybe a better connectivity. Or simply to spread his masternodes over different locations/data centers to make sure they won't go offline at once one day (for example, due to data center power outage or some another catastrophic event).
